This 55-year-old Virginia Beach woman was bothered by extra skin and fat bulges in her upper and lower lids. I had a very thorough consultation with her and explained that with careful inspection, we see not only a bulge beneath her lower lids, but there is another bulge further down and beneath the outside half of her eyes. However, this lower bulging area is caused by a roll of muscle, the orbicularis muscle. With a technique called a muscle hitch, this muscle is unfurled, tightened and suspended outside of the corner of the eye. This procedure, along with removal of the bulging fat and fat grafting, gave this patient a very smooth and younger appearance.
